Appointment Booking

POLO APPOINTMENT SYSTEM  
FOR OFWS REQUIRING VERIFICATION 
OF INDIVIDUAL EMPLOYMENT CONTRACTS


STEP 1 Please visit our website at www.polodoha.com.ph and click on Services and click on Appointment Online Booking. Fill-out the Electronic Application Form. Choose selected Date and Time for the appointment with POLO. Confirmation will be sent to your email.


STEP 2 Print email confirmation of Appointment Booking and bring printout on the assigned date and time with the following Requirements to the Philippine Overseas Labor Office (POLO) at this address: 1st Floor, Al-Jazeera Tower, Conference Center Street, Doha (Tower is beside the Ministry of Culture and Sports Bldg. across Shangri-La Hotel City Center). Only those with appointment for the day will be accommodated.

 

Requirements for Household Service Workers (HSWs)

(POEA MC No. 02, series of 2019 and POEA GBR No. 04, series of 2018)

  • Original HSW Employment Contract (employer should sign on all pages)
  • Two (2) copies of each document:
    • QID of an employer (front and back)
    • QID of worker (front and back)
    • Passport copy of worker (should be valid for at least 6 months from the date of intended departure from the Philippines)
  • Life and personal accident insurance of the worker issued by any insurance company in Qatar.
    • Legal basis: Refer to Section 10 of the model employment contract for household service workers (HSW)
  • 1 copy of Qatar Government issued Health Card
  • Original current Certificate of Employment signed by the sponsor/ employer
    • Indicates the name of the worker, date of commencement of work, actual position, and salary
  • **Sworn Statement/ Affidavit explanation stating the circumstances of the change of visa status or change of employer (if applicable)

NOTE: This requirement** only applies for the following individuals:

  1. HSWs who are not registered with POEA;
  2. HSWs who arrived in Qatar under tourist, family visit or business visa;
  3. HSWs whose company work visa was transferred to domestic worker visa;
  4. HSWs who transferred to another employer within Qatar

Requirements for Skilled, Professional and other Company Workers

(POEA MC No. 02, series of 2019 and POEA GBR No. 04, series of 2018)

  • Original signed Employment Contract with attestation/stamp by the Ministry of Labor (Government workers not required)
  • Two (2) copies of each document:
    • QID of worker (front and back)
    • Passport copy of worker (should be valid for at least 6 months from the date of intended departure from the Philippines)
  • Original Current Certificate of Employment signed by the sponsor/ employer, with company stamp
    • Indicates the name of the worker, date of commencement of work/ joining date, actual position, and salary
    • If not available, present valid company ID, or recent pay slips
  • *Sworn Statement/ Affidavit explanation stating the circumstances on how he/she was hired by the current employer (if applicable)**

 

NOTE: This requirement* only applies for the following individuals:

  1. Those who are not registered with POEA;
  2. Those who arrived in Qatar under tourist, family visit or business visa;
  3. Those who worked before from another country (except for the Philippines)
  • **Letter from the employer attesting to the transfer of job site

NOTE: This requirement** only applies for workers with existing POEA record who are returning to the same employer but to a different country
STEP 3 Upon reaching the Philippine Overseas Labor Office (POLO), fall in line at the Reception Desk, get your Number, and wait for your number to be called.
STEP 4 Payment of FEES. Verification of contract: QR 40.00

MEMBERSHIP REQUIREMENTS

  • Valid original passport & work permit
  • Any proof of employment such as verified employment contract, certificate of employment, company ID, pay slip, etc.
  • Accomplished OWWA Membership Form.
  • OFW must be physically present

MEMBERSHIP/RENEWAL FEE – QR  93.00 (Coverage is for a maximum period of two (2) years)

PROGRAM AND SERVICES

  • Assistance on OFW’s whereabouts, psycho-social counseling, settlement of complaints, airport assistance, legal assistance, work camps/hospital/deportation visits, community outreach
  • Assistance to runaways
  • Referrals for possible assistance under the “Balik-Pinay, Balik-Hanapbuhay” Project
  • Post Arrival Orientation Seminar (PAOS)
  • Reintegration Preparedness Programs
  • Seminars and Trainings on value formation, financial literacy, entrepreneurial development (EDT), techno-skills and capability–building (such as IT Program, Baking Lesson)
  • Special Operations (relief and rescue operations)
  • Repatriation program

ADDITIONAL INFORMATION:

  • Medical and Hospitalization Benefits are NOT
  • covered by OWWA. Company/PhilHealth provide these benefits to members.
